DUTIES & RESPONSIBILITIES:
• Work with resource parents providing foster care or adoption to children and youth in CPS care. Monitor the resource homes with in-home visits and phone calls and provide support and consultation services to resource families. Assist the families with completing Monthly Support and Monitoring Plans and review required documentation the family maintains for the children in their care.
• Assess the needs of each family and arrange for peer support, community resources or training as needed. Observe the general functioning of the home, meet individually on a regular basis with the CPS children in the home, and with all resource family members.
• Monitor the family’s participation in Child and Family Team meetings and support the family to communicate with CPS case managers and with the biological families of the children in placement, as indicated in the case plan.
• Respond to licensing inquiries, CPS referrals and other concerns. Work as part of a team to address formal complaints and inquiries. Identify resources needed to target deficiencies or problems, and ensure proscribed changes are carried out.
• Compile and maintain case records on all resource families according to Agency procedures. Document all family and collateral contacts. Review files regularly to make certain all relevant documents are present and in order and maintain timeliness in all documentation.
• Prepare periodic licensing or certification renewal reports and collect backup documentation within statutory and contractual time frames and send reports electronically or as mandated by contract to OLCR or County court.
• Work closely with the child’s case manager from DES to assure a smooth transition between child and resource parents.
• Assist with arranging support or training activities for assigned resource families.
• Attend DES meetings to obtain information about available children needing adoptive placements and work closely and in cooperation with DES case managers to assist in placement decisions; participate as a team member in reviewing prospective adoptive families and review requests for adoptive homes.
• Provide case managements services to adoptive families and assist adoptive parents in learning about children who may be available for adoption. Participate in CPS staffings when family is being considered for placement and provide support and consultation and refer adoptive families to other resources as needed to assure a smooth transition to placement between the parents and children. Complete subsidy applications as appropriate for each case, and files court paperwork to finalize the adoption.
• Assist resource families in bridging between foster care and adoption services as requested.
• Complete monthly billing and data reports according to supervisor’s specifications and respond to requests for statistical information.
• Perform other job-related duties deemed necessary and/or assigned by Supervisor.
QUALIFICATIONS/REQUIREMENTS:
• Must have knowledge and understanding of human behavior, social work theories and techniques; adoption process and laws and community resources.
• Bilingual Spanish and English required
• Must have familiarity with foster care.
• Must have skills in interviewing and case recording.
• Must have excellent communication and writing skills.
• Must be capable of intelligent, unbiased, independent activity, flexibility and good judgment.
• Must be able to meet deadlines and prioritize activities.
• Must recognize the uniqueness, values and identity of clients served and respect same. Must maintain appropriate personal boundaries with all clients served.
• Must be able to work with people in a variety of situations under various levels of stress while encountering a wide range of human problems.
• Must maintain strict confidentiality, at all times, according to agency policies and state law.
• Must have Bachelor’s of Social Work or closely related field
• Requires one or more years experience in child welfare.
• Must be able to obtain fingerprint clearance.
• Driving is an essential function for this position. Must have a valid AZ Driver License, Valid auto insurance and valid registration, and transportation.
SALARY: Grade 10 position. $29,500 to $32,195 annually depending upon experience.
